families and employees. We offer group life, health, disability, dental, vision, 401K and E&O.

FAQs
- Who can sign up? Any agent (new or existing) with National Brokerage, their dependents and their employees.
- How do I qualify? Read below for eligibility requirements.
- Who pays for the benefits? You pay for the benefits directly through Small Business United.
- When do I cancel my current benefits? ONLY after you have enrolled in the new benefits system AND you
receive your new policy via mail. Again, do not cancel your current coverage until you receive your new policy
that will be mailed to you.
- Can I pick and choose what benefits I want? Yes, absolutely. For example, if you only want dental coverage, you
only have to enroll for dental and nothing else.
- What if I don't want any benefits? You do not have to do anything at all. You will NOT be auto-enrolled.
You do NOT have to log in and waive all the benefits.
- When is open enrollment? See announcements for enrollment date updates.
- What is the deductible period? Calendar year. You can get credit for
deductible met in previous plan.
- Why do the plans contain the clause "This is not comprehensive major medical coverage..."? Medical
insurance is governed by each state's Department of Insurance. Some states only allow the use of the term
"major medical" under certain circumstances. Our nationwide group plan must comply with the strictest
standards of all 50 states in the description of the benefits.
- The health coverage annual maximum is too low, what can be done? Wrap coverage is now available with
much larger, customizable lifetime maximums.

Eligibility Requirements
In order to qualify for group benefits through National Brokerage you must meet
the following qualifications and guidelines:
- Complete and sign the Agent Benefits Pledge
and email or fax it to us: Email:

Note: A signed pledge is required from each writing agent that enrolls.
- Contract with a minimum of six life insurance carriers including Aviva, ING, Minnesota Life
and Transamerica, and contract with a minimum of three annuity carriers including Allianz Life Insurance Company and
Life Insurance Company of the Southwest (LWS).
- Produce of minimum of $15,000 in target life premium or $300,000 in annuity production
or $600,000 of face amount in life settlements, or any equal weighted combination,
in a 12 month period of time.
- Each writing agent that wants to enroll in the benefits must meet the qualifications by his/her own production.
- Each writing agents that qualifies for benefits can also enroll his/her dependents with no extra production requirements.
- Production requirements are multiple by the number of employees enrolled (employees exclude writing agents).
For example, if you have an agency with one writing agent and 2 staff members, the primary
agent must meet 3-times the normal production requirements if all 3 people are enrolled.

(mm/dd/yyyy).
